Overlook VNA- Home Care
The Home Care team gathers the best clinicians in every specialty to have a positive impact on your life paving the way to a prompt recovery. Nurses, Therapists (Physical, Occupational & Speech), Medical Social Workers and Home Health Aides will design a Plan of Care in coordination with your Physician to help you with the most complex clinical interventions while enjoying the privacy of your home.
We provide a variety of services through specially trained clinicians including:
- Nursing care
- Physical, occupational & speech therapy
- Medical social worker services
- Home Health Aides, homemakers and companion care
- Care management to coordinate any help you need
- Disease support programs
- Health monitoring though Telehealth, falls detection, personal emergency response systems and medication dispensing services
Overlook Hospice and Palliative Services has helped provide relief for Worcester County families for over a decade and recently has opened offices in West Springfield and Stoughton areas, expanding our coverage to Plymouth, Bristol, Hampden, Hampshire and Franklin counties.
From bereavement support and counseling, to pain management and integrated therapies to ease stress, we’ll work with you and your family during this tender time.
Our interdisciplinary team includes a physician who acts as a resource for attending physicians, assuring quality patient care and symptom management. Our registered nurse leads and supervises the team while supporting patients and caregivers. Our social worker provides counseling, therapy, community resource planning and grief counseling. Spiritual care is also offered in your chosen faith. And carefully trained and committed volunteers will be right by your side to provide respite, companionship and assistance.
