Adult Day Program
The Overlook Vista Adult Day Program or The Vista Club, provides programming and social engagement for adults with Alzheimer’s or other forms of dementia. The Vista Care Team designs the activities to help members connect, form friendships, and create positive moments that uplift members.
Caregivers receive the benefit of respite to relax and recharge. They can focus on their needs knowing their loved one is in a caring and safe environment.
Care Management
You deserve a health care plan that takes all of your unique needs into consideration. Clinical social workers and nurses are our care managers and specialize in providing high-quality health care to older adults.

Enhanced Living
Residents who enjoy our Enhanced Living option receive assistance with activities of daily living so they have the support they need to continue to live independently and stay as active as possible.

After a professional Overlook Visiting Nurse Association team creates a care plan, you or your loved one will receive the care and services you need to continue to age in place. Some examples of services include:

Through a physician’s referral, patients can receive hospice care or palliative care at The Overlook. These forms of care are different. The Overlook serves both residents and people in the greater surrounding community. Non-residents who are qualifying patients can get the hospice and palliative care they need at The Overlook.

Located on our campus, Fieldstone at The Overlook is home to residents with Alzheimer’s or other forms of memory loss. Staffed with a compassionate group of nurses and Resident Care Assistants, residents at Fieldstone receive memory care in a safe environment focused on providing them with comfort, familiarity, and purpose.
Private Duty Home Care
Private care services help support our residents by providing them with the care they need in their own homes. The Overlook offers a wide variety of services to support individual needs.
Ranging from clinical support, personal support, and community involvement, private care services at The Overlook will address your care plan development; assistance with managing the care plan; support in the home, like getting dressed, maintaining nutrition, and housekeeping; and participating in activities to stay active and try something new.
Short-Term Rehabilitation
The Overlook’s rehabilitation center offers services seven days a week. Our rehabilitation care team will work with you to make sure your needs are met. We address post-acute needs like:
